Root Cause Analysis

Overview

Root Cause Analysis is a vital process and a systematic path for identifying the Root Cause of problems. There are many approaches that attempt to discover the root cause of issues, including Visual Brainstorming, the 5-Why Approach, the KJ Method, or the usual questioning method. However, these are not the most effective methods.

Root Cause Analysis is an investigative approach based on Patterns of Clues derived from symptoms or the phenomena of occurrence.

Therefore, for every problem, there are many possible physical causes. The investigator must be able to correctly define and formulate the problem so that a shorter and more direct path can be taken to diagnose the problem with focused approach to drill down to the root cause.
