Root Cause Analysis
Overview
Root Cause Analysis is a vital process and a systematic path for identifying the Root Cause of problems. There are many approaches that attempt to discover the root cause of issues, including Visual Brainstorming, the 5-Why Approach, the KJ Method, or the usual questioning method. However, these are not the most effective methods.
Root Cause Analysis is an investigative approach based on Patterns of Clues derived from symptoms or the phenomena of occurrence.
Therefore, for every problem, there are many possible physical causes. The investigator must be able to correctly define and formulate the problem so that a shorter and more direct path can be taken to diagnose the problem with focused approach to drill down to the root cause.
What Will Be Covered
1. Understanding of the many types of problems and the ways to resolve them.
2. Root Cause Mapping terminology covering Physical Roots, Human Roots, and Latent Roots.
3. How to define and formulate problems in terms of their respective key elements.
4. How to identify Primary Effects, Escape Points, Contributing Causes, and Root Causes.
5. Investigative approach based on diagnostic methods for each problem type.
6. Identification of Change Components, Separation Logic, and Focused Questioning Approach.
7. How to look for causes using the Swapping Method, Switch Method, Performance Range Method, and Probability Method.
8. Simple Statistical Method to verify the root cause.
9. Prove the root cause.
How Is The Training Conducted
The 2-Day Root Cause Analysis training involves many problem-solving exercises to develop the participants’ familiarity with the technique. Upon completion, the participants will become better at problem-solving to:
a) Improve process quality
b) Solve operational problems without relying on statistics
c) Solve process problems without relying on slow data-collection and too much statistical calculation
d) Troubleshoot problems and identify the root causes
e) Solve problems quickly with effective results
